Low testosterone, also called testosterone deficiency or male hypogonadism, occurs when the body does not produce enough testosterone to support normal physiological function.
A testosterone number alone does not establish the diagnosis. Results must be interpreted alongside symptoms, health history, testing conditions, laboratory reference ranges, and possible underlying causes.
These symptoms are not specific to testosterone deficiency. Sleep problems, thyroid disorders, depression, medication effects, obesity, chronic illness, relationship factors, and other conditions can produce similar symptoms.

If fatigue, libido, sleep, or strength changes prompted the question, start with the Low-T symptoms men often notice in their 40s and when they warrant an evaluation.
Not every patient requires every test. Laboratory selection depends on symptoms, age, medical history, previous treatment, fertility goals, and physician judgment.
Estradiol has normal roles in men. An estrogen blocker is not an automatic part of TRT; discuss symptoms and laboratory findings with the prescriber before adding or changing medication.
No. A total testosterone result near or below 300 ng/dL is commonly considered during clinical evaluation, but it is not an automatic diagnosis or prescription threshold. The result should be considered with symptoms, repeat testing, assay accuracy, free testosterone when appropriate, and possible causes of suppression.

When appropriate, Dr. Mackey follows current Endocrine Society and American Urological Association recommendations, which may include repeat morning testosterone testing before initiating therapy.

Injectable testosterone is a commonly used treatment option that allows the dose and schedule to be adjusted according to laboratory findings, symptoms, and treatment response.
The dose is individualized rather than selected solely from age, body weight, or one testosterone result.

Topical testosterone may be appropriate for patients who prefer a non-injectable option. Patients must follow instructions intended to reduce accidental transfer to partners, children, or other household members. Application location, skin contact, handwashing, clothing, and bathing instructions should be followed carefully. Absorption can vary between individuals, making follow-up testing important.

Oral testosterone offers an alternative for selected patients who do not want injections or topical treatment. Its suitability depends on medical history, cardiovascular risk, blood pressure, medication interactions, treatment goals, cost, and monitoring needs.

Enclomiphene and clomiphene may stimulate hormonal signaling involved in the body’s natural testosterone production. They may be considered for selected men, particularly when preserving testicular function or fertility is an important concern.
NovaGenix generally prefers enclomiphene when appropriate because it may offer advantages for some patients, including a potentially more favorable side-effect profile. However, NovaGenix may also prescribe clomiphene, commonly known as Clomid, when a patient is interested and the physician determines that it is medically appropriate.
Individual results vary, and neither medication is suitable for every cause of low testosterone.
External testosterone can suppress the hormonal signals needed for sperm production. Men who currently want children or may want children in the future should discuss this before beginning TRT.

NovaGenix may help treat hormone-related factors that can affect male fertility, but treatment cannot guarantee improved sperm count, improved semen quality, fertility preservation, fertility recovery, or conception.

In appropriately diagnosed patients, testosterone therapy may improve certain symptoms or health measures.
Responses involving energy, mood, erectile function, strength, body fat, cognition, or exercise performance vary considerably. TRT should not be presented as a guaranteed solution for every symptom.

The TRAVERSE trial enrolled 5,246 men ages 45 to 80 who had symptoms, two fasting testosterone results below 300 ng/dL, and preexisting cardiovascular disease or elevated cardiovascular risk. The primary major cardiovascular outcome occurred at similar rates in the testosterone and placebo groups and met the study’s noninferiority standard. These findings should not be generalized to every patient, formulation, dose, or treatment duration. TRT should not be described as universally risk-free or universally cardiovascularly safe.
Testosterone can increase red-blood-cell production and hematocrit.
Patients should not change their medication or independently arrange blood donation as a substitute for physician evaluation.

Blood pressure should be considered before and during treatment, especially in patients with hypertension, cardiovascular disease, kidney disease, multiple cardiovascular risk factors, or medications that affect blood pressure.

THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.
