GLP-1 Medications: Semaglutide and Tirzepatide

Semaglutide and tirzepatide are the two most extensively studied medications currently used for weight management. Semaglutide is a GLP-1 receptor agonist; tirzepatide acts on two receptors, GLP-1 and GIP. Both are given as a once-weekly subcutaneous injection and both are started at a low dose and increased gradually rather than begun at a maintenance dose. What they do:

• Slowing digestion so you feel full longer
• Supporting steady, sustainable fat loss

In the published trials, semaglutide 2.4 mg with lifestyle intervention produced a mean reduction of 14.9% of body weight at 68 weeks (STEP 1), and tirzepatide produced mean reductions of 15.0%, 19.5% and 20.9% at 72 weeks at 5 mg, 10 mg and 15 mg respectively (SURMOUNT-1). Those are trial averages under trial conditions. Lipotropic and B12 injections

Peptide therapy for metabolic support

Compounded topical therapy

Some patients are prescribed a compounded aminophylline and glycyrrhetinic acid topical as an adjunct to a weight-loss plan, for localized areas.

A 2023 systematic review in Frontiers in Endocrinology pooled five clinical trials of topical aminophylline and found greater circumference reduction at the treated site than at the untreated control site in four of the five. Skin rash was the main side effect reported. The review’s authors note that the evidence base is small — 117 participants across five trials, several of them decades old, with a high risk of bias in some — and call for further high-quality trials.

Topical aminophylline is not FDA-approved for fat reduction; aminophylline’s FDA approval is as a prescription drug for asthma. Short-term appetite suppressants

Phentermine and similar appetite suppressants are prescribed for short-term use in selected patients, and are not appropriate for everyone — particularly patients with certain cardiovascular conditions.

Nutrition and activity

Medication is one part of the plan. Nutrition and activity guidance is individualized to your labs, your schedule and what you will actually sustain. Who is a candidate for medical weight loss and who is not

Adults generally qualify for prescription weight-loss medication with:

• a BMI of 30 or higher, or
• a BMI of 27 or higher with at least one weight-related condition — high blood pressure, high cholesterol, type 2 diabetes, cardiovascular disease or obstructive sleep apnea.

These are the criteria the FDA uses for medications including Wegovy and Zepbound. Final eligibility depends on Dr. Mackey’s evaluation, your medical history, your contraindications and the specific medication being considered. Meeting the BMI criteria does not by itself mean a medication will be prescribed.

GLP-1 medications are not appropriate for patients with:

• A personal or family history of medullary thyroid carcinoma
• Multiple Endocrine Neoplasia syndrome type 2
• A previous serious allergic reaction to the medication
• Current pregnancy

Certain gastrointestinal, pancreatic, gallbladder, kidney and eye conditions may also affect whether a medication is appropriate, and patients taking insulin or other diabetes medications need careful coordination because of hypoglycemia risk. Side effects you should know about before you start

The most common side effects of GLP-1 medications are gastrointestinal: nausea, diarrhea, vomiting, constipation and abdominal discomfort. These are most likely during dose increases. More serious risks are possible, and your clinician will review the full medication-specific prescribing information with you and explain when to seek medical attention.
