
Testosterone therapy requires a diagnosis, discussion of benefits and risks, and a follow-up plan. A clinic cannot guarantee life-changing results or risk-free treatment. Choose a clinician who explains why a treatment is appropriate and how its value will be reassessed.
Hypogonadism can involve the testes or the hormonal signals that regulate them. Men need compatible symptoms and consistently low testosterone confirmed with separate morning tests. Treatment is individualized; it is not automatically required for every low result.
Ask how the clinician evaluates contributing illnesses, medications and fertility plans. Other treatments or specialist evaluation may be more appropriate in some circumstances.
TRT should not be promoted as preventing Alzheimer's disease, cancer, heart attacks or diabetes. Nor should a patient be promised improved work performance, stronger attraction, better sleep or a particular physique.
The AUA guideline describes potential improvements in sexual symptoms, anemia, bone density, lean mass and depressive symptoms, while evidence for several other outcomes is inconclusive. A potential benefit is not a guarantee for an individual patient.
Testosterone can raise hematocrit and blood pressure, suppress sperm production and cause other adverse effects. Calling side effects “very rare” without context understates the need for monitoring. The FDA's 2025 labeling update added blood-pressure warnings across testosterone products.
Before treatment, discuss relevant cancer history, sleep apnea, cardiovascular conditions and other health concerns. Active prostate or breast cancer and certain other conditions require avoiding TRT or further specialist assessment. Monitoring does not eliminate every risk.
