Doctors & Clinics That Prescribe hCG for Men

hCG is a prescription medication with legitimate uses in male reproductive medicine. The right clinician should evaluate why it is being considered—not simply add it automatically to every testosterone protocol.

Why do doctors prescribe hCG to men?

hCG binds to the same testicular receptor stimulated by luteinizing hormone (LH). In men whose testes can respond, this can support intratesticular testosterone production. The AUA/ASRM male infertility guideline recognizes hCG as one option clinicians may use in selected infertile men with low serum testosterone and describes gonadotropin treatment as an established approach for men with hypogonadotropic hypogonadism.

hCG should not be marketed as a universal requirement for every man with low testosterone or every man receiving testosterone replacement therapy.

hCG, TRT and fertility

Exogenous testosterone suppresses hypothalamic and pituitary signaling, including LH and FSH. As a result, sperm production may decline substantially or stop. The 2024 AUA/ASRM guideline advises against prescribing exogenous testosterone to men interested in current or future fertility. Adding hCG or a SERM does not remove that concern; evidence for preserving fertility with such combinations is too limited to recommend this approach.

When fertility is important, evaluation should focus on the actual reproductive goal. Depending on the situation, this can include reproductive history, semen analysis, testosterone, LH, FSH and other testing. hCG, SERMs or other fertility-directed therapies may be considered according to the diagnosis.

Can a telehealth clinic prescribe hCG?

A telehealth clinician may be able to prescribe hCG when permitted by applicable federal and state law, the clinician is appropriately licensed for the patient, a legitimate clinician-patient relationship has been established, and the medication is medically indicated. Telemedicine does not eliminate the need for proper evaluation, documentation, laboratory testing when appropriate, prescribing standards or pharmacy requirements.

Availability can also vary by state and pharmacy. A reputable clinic should never guarantee a prescription before medical evaluation.

What changed with compounded hCG?

In March 2020, certain previously approved biological products transitioned to regulation under the Public Health Service Act. FDA specifically identified human chorionic gonadotropin among the products affected. These biological products are not eligible for the same 503A and 503B compounding exemptions that apply to eligible compounded drugs.

That regulatory change altered the compounding landscape, but it does not mean that hCG itself became illegal or ceased to exist. FDA-approved hCG products remain prescription medications. Patients should obtain hCG only through legitimate prescribing and pharmacy channels.

How to choose a clinic that prescribes hCG

Look for a clinic that evaluates the reason hCG is being considered rather than selling it as an automatic package add-on. The clinician should discuss fertility goals, prior testosterone or anabolic-steroid exposure, symptoms, relevant medical history, laboratory results and monitoring.

If fertility is the primary concern, semen analysis and/or referral to a reproductive urologist may be more important than simply adding another medication.

Do I need a prescription for hCG?

Yes. hCG is a prescription medication. It should be prescribed by an appropriately licensed clinician after determining that its use is medically appropriate.

Can any doctor prescribe hCG?

Prescribing authority depends on professional licensure, jurisdiction and applicable law. More importantly, the clinician should be qualified to evaluate the condition for which hCG is being considered.

Can hCG help preserve fertility during testosterone therapy?

hCG has a role in male reproductive medicine and can stimulate testicular testosterone production, but fertility management is individualized. Men actively trying to conceive should not assume that adding hCG to testosterone guarantees preserved sperm production.

Is hCG the same as gonadorelin?

Can I buy hCG online without a prescription?

Patients should not purchase prescription hCG from websites that bypass legitimate prescribing and pharmacy requirements. The safer route is evaluation by an appropriately licensed clinician and dispensing through a lawful pharmacy.
